- Mission of Establishment
- Long Term Care becomes a common issue with the aging population. The number of people who need Long Term Care also increased. Lienchiang County Long Term Care Management Demonstration Center established in July 2003 providing residents free social welfare and related resources counseling in one-stop window, anticipating elderly and disabled people can obtain appropriate medical care and day-to-day care.
- Service Object
- Elderly over age over 65 and cannot attend to themselves
- People who need Long Term Care (i.e., disabled, dementia, mental disorder, and incapable to attend to themselves and need Long Term Care and home services) and their family
- Residents who need counseling in related fields
- Services
- Referral service：understand and assess the needs of the case in Long Term Care. Assist them to choose the appropriate services and resources, i.e., home services, home nursing, nursing institution or elderly welfare institution.
- Social welfare resources counseling: gather all log nursing institutions, manpower and social welfare resources in the county. In order to understand the current situation, provide counseling services through one-stop window.
- Case management: evaluate the needs of each case. Help them refer to appropriate resources. Seek related subsidy. There is a case manager following up with every case from receiving, referral to case close.
- Safe living aids display and rental: old and worn living aids recycle and rental services. Telephone counseling and greetings.
